Request Workflow
You are welcome to contact us for the sample catalogue. Once the desired sample type is determined, you can discuss with our Operation Team regarding the availability of the desired samples. After mutual agreement is reached, you can submit the sample request application form.
CU-Med Biobank Access & Release Committee will evaluate your application based on different criteria, including sample quantity, the preference of sample depositor, scientific merit and donor welfare. The application result will be informed in due course.
Upon approval of the request application, sample release procedures may commence.
For CUHK sample requestors
We will inform you as soon as the requested samples are ready. The request team picks up the samples from CU-Med Biobank, and you are required to fill-in and submit the Sample Retrieval and Check-out Record.
For non-CUHK sample requestors:
We will inform you as soon as the requested samples are ready. Before picking up the samples, you are required to sign the material transfer agreement (MTA) form. If you are not available to pick up the sample in person, we can discuss the pick-up method, e.g. By courier (Requestor is responsible for the delivery charge). You are required to fill-in and submit the Sample Retrieval and Check-out Record.

Acknowledge CU-Med Biobank in Publications
Notify CU-Med Biobank
Charge will be applied for checking out sample from CU-Med Biobank. You may request us to send you a Request Application Form for more details.
Once the samples are released, CU-Med Biobank will issue an invoice to the sample requestors. Payment can be settled by interdepartmental transfer (for CUHK requestors only) or by cheque.
Scientific research can then be conducted after receiving the samples. If any research results are published using our samples, researchers are required to acknowledge CU-Med Biobank in the publication and notify us.
